The SBL Bonutti Clinic rolls out Chronic Care Management and Remote Patient Monitoring to orthopedic patients. This extension of care is made possible through to a state-of-the-art digital engagement platform and easy-to-use wearables provided at no cost to Medicare patients.
A brand new medical service with the power to transform patient care is being implemented in the Effingham region.
Sarah Bush Lincoln has partnered with Ceras Health to bring Remote Patient Monitoring and Chronic Care Management to the Bonutti Clinic. Ceras Health is an industry leader in patient engagement, providing intuitive digital applications with easy-to-use interfaces, wearable devices with straight-forward controls, and instant, ongoing remote access to care teams.

Patients undergoing joint repairs have the option to wear a HIPAA-compliant, always-on, smart watch that monitors heart rate, pulse, temperature, blood oxygenation, mobility and more. It is available at no cost to Medicare recipients. If readings aren’t where they need to be, a clinical expert can intervene and, if necessary, schedule an immediate follow-up appointment with the patient’s doctor. Dr. Bonutti added, “Outpatient surgery will significantly benefit from monitoring with safety and the ability to notify patients at home.”
Supported by the Ceras Health platform, Remote Patient Monitoring, or RPM, will enable the staff at the SBL Bonutti Clinic to track orthopedic patients’ recovery from surgery and their ongoing progress without the need to come in for unnecessary repeat visits.
In addition, patients suffering from two or more chronic conditions will benefit from Chronic Care Management, or CCM, which provides customized, evidence-based care plans designed to support wellness and limit risks associated with a variety of chronic conditions. As with RPM, patients on a CCM plan will benefit from ongoing monitoring from a team of clinical experts who can check in at regular intervals and ensure health remains on track by updating the care plan and scheduling follow-up appointments as needed.
Data remains secure and fully confidential, accessible only to the patient, their doctor and Ceras Health’s dedicated team of clinical professionals.
This marks the SBL Bonutti Clinic’s latest move to prioritize the patient experience and innovate care within Effingham and the surrounding community. Work recently began on a brand-new facility that will expand care options and bring a variety of specialties together under one roof. Combined with the remote engagement services made possible by Ceras Health, the Bonutti Clinic continues to explore new and exciting methods of supporting patient wellbeing.
